Studies suggest that health plans that administer Part D will increase financial exposure because of the reforms Congress implemented in 2022 that were meant to help patients. PBMs and their health plans are already blunting their financial impact by increasing average out-of-pocket costs by 32% in 2025 for medications that have had their price capped by Medicare. In short, the savings are not being passed on to the people who need them.

I am concerned about the negative impact the Medicare Drug Price Negotiation Program could have on medication access & patient out-of-pocket costs for Medicare beneficiaries.

Cardiovascular disease remains America’s number 1 killer, claiming the lives of over 700,000 adults annually. Staggeringly, 42% of Medicare Part D beneficiaries suffer from at least one heart condition. Over the past 15 years, the death rate due to cardiovascular disease has increased, undoing 50 years of progress.

Research suggests that pharmacy benefit managers who administer Part D plans will have higher financial exposure because of Maximum Fair Price negotiations & Part D redesign. To blunt this, PBMs & their plan sponsors will increase out-of-pocket costs for patients & limit access. This is already happening, as average out-of-pocket costs for patients on MFP medications increased by 32% in 2025.

We commend efforts by Congress & CMS toward Medicare's Part D redesign, but unless out-of-pocket costs are decreased, the impact of these negotiations will be meaningless – & potentially harmful - to beneficiaries.

Please continue to hold CMS accountable to ensure these provisions are implemented thoughtfully & effectively. Only CMS can guarantee that plan sponsors & their counterpart PBMs do not increase the out-of-pocket costs & administrative burden on those who benefit from these medications.
